Waiver Form

You must sign this Waiver Form in order to participate in this study / research project (“Study”).
If you need additional explanations, consult any of the recruiter you are dealing with.

About us: GigaHertz is an independent research services company. We conduct researches in Brazil for the below mentioned clients, sponsor companies and their ecosystem (“Clients”) in several countries.

Study documentation: all Study’s sessions and activities will be recorded in audio, photo and video to register your participation (“Study Data”). We share all Study Data with he Study’s Clients.

Study Data will be used for:

  • Research analysis purposes;

  • Training purposes;

  • To generate reports for Clients;

  • Internal strategy use by the Clients.

Yours rights over your data:

  • To be informed about how it will be used;

  • Access, copy, update, correct or delete your data at any time;

  • Contest the collection and / or use of your data to complain to the relevant authority;

  • Request changes to your tillering.

  • If you want to access, copy, update, correct or delete your data contact us at pesquisas@ghz.rocks;

  • There are no penalties for withdrawing this Waiver and you are free to do so at any time.

Confidentiality:

  • All of your information and images will be kept confidential, no private details will be revealed and security measures will be adopted to keep the information secure. We and the Clients will take precautions to ensure your anonymity and keep your identity confidential.

  • The full real names of the participants are not used in the documents generated by the study; we and Clients only use the participant's first name or pseudonym.

Our Clients and all eventual third parties involved in Study are also obliged to treat all materials, using them anonymously, confidentially and will not publish or distribute them outside of their businesses, without their express written consent.

Secrecy is important:

By signing this term, you acknowledge that during Study’s activities Clients may share information or concepts of confidential or proprietary nature. You agree to maintain confidentiality and not to publish or disclose any information regarding this Study to third parties and to adopt the same degree of care to safeguard it as you do to protect your own personal information.

By signing the agreement you agree that:

  • Your participation in the Study is completely voluntary and you are free to terminate your participation at any time if you wish.

  • You have read, understood and agree to the conditions described.

  • You understand that we will take precautions to keep your identity confidential.

  • You acquit GigaHertz, Clients and both partes employees and directors from any actions, damages, claims and demands that you may have against GigaHertz and its Clients arising or resulting from my participation in this Study.

  • GigaHertz is the only company involved in this Study authorised to contact you. Under no circumstances will Clients be able or allowed to contact you directly.

  • We use the digital signature method through an application. You will automatically receive a copy of this term.

  • You are being provided with a monetary award as an incentive for you participation, with a previously agreed amount.
